Release of Information (ROI) Form

This Release of Information (ROI) form allows the West County Salesian Youth Club (WCSYC) to share relevant information regarding club members with WCSYC staff, who serve as case managers. This information helps monitor members’ attendance, behavior, and academic performance, provided permission is granted by the parent/guardian.
